A Cochrane review of two small one-year trials in boys with fragile X syndrome found low-quality evidence that oral L-acetylcarnitine (LAC) does not clearly improve intellectual functioning or hyperactive behaviour compared with placebo.
What the researchers did
Authors searched for randomised trials of any-dose LAC versus placebo in people with fragile X. Only two trials (83 boys, ages 6–13, mean about 9) met inclusion; both lasted one year. Outcomes included Wechsler IQ scores, Conners hyperactivity ratings, and Vineland adaptive behaviour. Evidence quality was rated with GRADE. Fragile X often overlaps with autism-spectrum presentations, which is why this review matters for many NeuroDifferent readers.
What they found
- No important LAC-versus-placebo differences in verbal or non-verbal IQ were reported (detailed numbers not provided by trial authors).
- Teacher Conners scores showed no clear difference in one trial (mean difference 0.50, 95% CI −5.08 to 6.08, n = 51; low-quality evidence).
- Parent hyperactivity ratings favoured LAC in one tiny trial but not the larger one; changes were not judged clinically meaningful.
- Vineland composite was unclear overall; the socialisation domain favoured LAC in one analysis (mean difference 11.30, 95% CI 2.52 to 20.08, n = 51; low-quality evidence).
- No side effects were recorded in either trial.
What this means for families and therapists
- Current evidence does not support LAC as a reliable way to raise IQ or settle hyperactivity in fragile X.
- If LAC is still discussed, ask clinicians what outcome they expect and how it will be measured against behavioural supports already in place.
- Behavioural and educational interventions remain the more established levers while this supplement evidence stays thin.
Limitations and what we don't know yet
- Only two small all-male trials; girls and adults unstudied here.
- Unclear randomisation, allocation concealment, and outcome blinding in places; industry involvement raised bias concerns.
